Please complete the below Order Form if you are interested in purchasing Pre-Sale Items for MSPTSA's Spring Carnival.

Orders & payments MUST BE received by March 3rd. 

Fulfilled orders will be sent home with your student on March 5th.

After submission of this form, you will be invoiced via PayPal.  You can pay with your PayPal account or a Debit/Credit card.  Convenience fees will be applied on the invoice, or you can submit a check/cash by sending an envelope to school with your student labeled "Carnival Pre-Sale".
